Please be aware that it is my responsibility, as a rescue person, to place these cats in the most caring and loving homes I can find for them. Most have been through horrible abuse and neglect and deserve PERMANENT homes where they will be properly cared for. Part of "properly cared for" means regular visits to the vet. This means that I DO REQUIRE information from your vet that your current pets are up-to-date on examinations, vaccinations and are spayed/neutered. If you have objections to this, please do not fill out this application. 1. Name, address, and phone number, email if applicable. 2. Yourvet's name and phone #. 3. Household members' names and ages. 4. Do you own or rent? 5. If you rent, what is your landlord's name and phone #? 6. Current pets, including breed , age, sex. 7. Are these pets spayed/neutered, and current on vaccinations? 8. Please list any previous pets you have had in the last 5 years and what happened to them. 9. Have you surrendered any animals to a local shelter or rescue? Why? 10. What breed of cat are you most interested in? Why? 11. Describe any special needs this breed might require. 12. Do you plan to declaw? Why? 13. Will the cat be indoor or outside? 14. Are you open to considering a special-needs cat (one with possibly a deformity, older, abused, etc.)? 15. Please list anything you would like to include regarding your experience with animals, especially cats and this breed (if applicable). we appreciate your taking the time to fill out this application. We know that you are aware of the plight of animals in shelters and abusive homes and that you appreciate the care we take in finding the best homes possible. Thank you. Adoption fees: Vary for individual animals. Contract: Adoptive owners must sign a contract agreeing to provide a home as specified in the application and, should it ever become necessary to give up an adopted cat, said cat will be returned to Last Resort Cat Rescue rather than a shelter.
